Does it affect everything or just organic stuff?
What about titans & bio titans, how does they react to this?

Well, it is an Etherial attack, and doesn't say no...so yeah, I'd say it affects anything that isn't specifically "Inorganic" (eg Necrons/Robots/Wraithguard). I'd guess for a normal titan, psychic save and if failed a hit against the head, normal save applies. For a Bio Titan...hmm....psychic save, if failed roll for armour and if failed roll on Head Critical Table would be my interpretation.
